OR FOR MAIL IN ENTRIES:

• Coaches should use the team reservation form.

• After completing the information on the bottom half of the page, check Lacrosse, Adult in the “Sport Entered” section, and enter the appropriate event code and division.

• Reservation form and fee must be received by the Powerade State Games office on or before May 29.

• After your team reservation form and fee are received, you will receive a confirmation of your entry which will include more specific information.

SPECIAL INFORMATION: Teams will be guaranteed a minimum of 3 matches.

RULES: 2010 U.S. Lacrosse rules will apply. Please visit www.lacrosse.org for the official rulebook. The following rules supplement the rules listed on the website.

• The match will consist of four 10 min. running time quarters.

• No horns.

• No time counts except for the goalie.

• 5 longpoles maximum.

• Penalty time is served.

• If the match ends in a tie, sudden death overtime will decide the match.

FORMAT: The format will be decided based on the number of teams who enter. All teams will be guaranteed 3 matches.

AWARDS: Gold, Silver, and Bronze medals will be given to the 1st, 2nd, and 3rd place teams respectively in each division. Only those coaches and players listed on the official roster will receive medals.
